- Assessment completed by Arbor View Administration
- “Pre-Admission Health Examination” completed by current Physician
- History & Physical
- TB Skin test / chest X-ray
- Free of communicable disease
- Signed “Standing Order”
- MD order to administer medications
- Special orders pertaining to diet, exercise, skin, etc.
- POA documentation
- Advance Directives, DNR, & Living Will documentation
- Copies of Insurance cards
- First and last month’s payment due upon move in date
